The perpetrator, the polluter, or sometimes a government agency can use the uncertainties of the science to deny to the courts or to the court of public opinion that any harm is being done. Government officials tend to react more swiftly and exercise precaution when the victims of damage are well identified - such as victims of infectious disease or accidents - and when cause and effect links are clear. The same is true of members of the the public, who may be moved to support gun control, for example, because the danger is clear, even if gun violence may never affect them individually. We have even gained protections for those who have no voice in the process, such as plant and animal species near extinction. The causes of species decline are known, the victims are known, and therefore protections are fought for and won. Even in these cases, action takes place only long after the victimization has begun, damage has occurred, and many have suffered. And when the victims remain unproven or unidentified, full health protections for people or ecosystems are rarely achieved under our current system. Various analysts have made this flaw in democratic process starkly clear. Merrell and Van Strum 1990, as cited in O'Brien, 2000, p. 80 ; posed the following question about pesticide risks: "What would happen if our pesticide risk assessments were so scientifically advanced that assessors knew not only exactly how many people would be harmed by a particular pesticide but also which individuals would be harmed? A permit to release a carcinogenic pesticide into the food system, for instance, would be preceded by a list of those who would contract the cancer. This, however, would constitute premeditated murder. The marked people would be entitled to an injunction on using that pesticide by their Constitutional right to life. However, bureaucrats and the private sector routinely 'get away' with this premeditated murder because the victims are individually anonymous." The fact that our current system allows unidentified individuals to be harmed without their knowledge or consent results in the routine violation of the individual's right to life, liberty, and the pursuit of happiness and our collective right to health and safety. The precautionary principle is necessary to guarantee this basic right. It says that if there is credible reason to believe that the actions of one entity will hurt another, action should be taken to protect the potential victims. Why Not Leave It to the Experts? Human activities cause some harm to ecosystems, and could harm human health. We can't live without risk. We need a process to decide the safest ways to meet our needs. When do the benefits of an activity outweigh the risks? The U.S. process for arriving at those decisions is based on a system of elected officials, protective laws, technical experts, risk assessments, cost-benefit analysis, public hearings, and stakeholder input. Daniel Fiorino 1990 ; notes: "Many observers argue that risk decisions are best left to administrative officials in concert with scientific experts, acting under instructions from elected representatives, and consulting as necessary with interest groups representing aggregated 'public' interests Elites, it is argued, will make more rational decisions." Proponents of the precautionary principle see a need for a new system of democratic methods that protect individual, collective, and minority rights. Mary O'Brien 2000, p. 79 ; states: "While permitting hazardous activities is unavoidable to some degree in a representative democracy of 250 million citizens, we must look at the degree to which communities are requiring and allowing government and business to pronounce the adverse effects of.
CASE REPORT: INTRAOPERATIVE NASAL BLEEDING COMPLICATION irreversible platelet-to-platelet aggregation Schlienger et al., 2003 ; . Celada et al. showed a decrease of platelet serotonin levels by 89% and plasma serotonin levels by 60% after a 12 weeks administration of 100-150 mg fluvoxamine per day in depressed patients n 11 ; Celada et al., 1992 ; . Pollock et al. additionally investigated the effect of paroxetine on the platelet release products PF4 and -TG in depressed patients during a 6 week treatment n 10 ; . these patients, mean PF4 and -TG decreased after 1 week and remained low after 3 and 6 weeks of treatment with paroxetine Pollock et al., 2000 ; . However, clinically significant bleeding time prolongations may mainly develop in patients with preexisting platelet abnormalities, since reports of bleeding complications in SSRI treatment remain rare Skop et al., 1996 ; . Previously published single cases of bleeding complications in SSRI treatment report varying laboratory data. Cooper et al. observed normal platelet count, prothrombin time, international normalized ratio INR ; , partial thromboplastin time PTT ; and bleeding time in a case of ekchymoses after 15 days of treatment with 20 mg fluoxetine per day Cooper et al., 1998 ; . Wilmshurst et al., who report a case of eye haemorrhage after 1 week of treatment with 20 mg fluoxetine per day, found a normal blood count including platelet count ; Wilmshurst et al., 1996 ; . In another case of peripheral ecchymoses reported by Calhoun et al., a repeatedly verified prolonged bleeding time of 12.5 minutes normal 2, 5-9, 5 minutes ; after a 4 month treatment with 50 mg sertraline per day Calhoun et al., 1996 ; . In another case of ecchymoses of the legs after a 2 week treatment with 20 fluoxetine per day, Ottervanger et al. found bleeding time, platelet count, PTT, and thromboplastin time to be within the normal range. Platelet aggregation with ristocetin was normal, but platelet aggregation with addition of epinephrin was elevated Ottervanger et al., 1994 ; . partially contained by tamponades. At about 36 hours after surgery a heavy haemorrhage redeveloped in the treated area, accompanied by hypertensive blood pressures. These heavy haemorrhages persisted for several hours and returned for at least once a day on the following days. It was attempted to stop the bleeding with the administration of antifibrinolytic tranexamic acid 4 x 500 mg on day 1 after surgery and 500 mg on day 2 after surgery ; . These attempts paused the bleeding for a maximum of 1-2 hours respectively. The blood pressure was at mildly elevated levels post surgery, but they never exceeded 150 100 mmHg. A treatment with 10 mg amlodipine per day was started on day 4 after surgery. Under treatment with amlodipine, blood pressure levels were normalized. On day 2 after surgery, a complete status of the blood clotting system was determined. Again, all standard clotting parameters were within the reference range. Hemoglobine was at 12.8 g dl, INR was at 0.93, PTT was at 34.6 and thrombine time was at 16.2 seconds. However, in this more detailed examination we found clear indicators for an impaired thrombocyte aggregation. Thrombocyte aggregation TA ; with collagen 1.0 g ml was at 70% reference 7095% ; , TA with arachidonic acid 1 mM was at 3% reference 70-95% ; and TA with epinephrin 8.0 M was at 25% reference 60-95% ; . Thrombocyte desaggregation TD ; with ADP 1.0 M l was at 85% reference 0-50% ; , TD with ADP 2.0 M l was at 91% reference 0-50% ; and TD with ADP 5.0 M l was at 50% reference 0-30% ; . The haemorrhages terminated on day 5 after the surgery, partly attributed to the successful treatment of the mild hypertension with amlodipine. Hemoglobine measured the lowest at day 7 after the surgery 9.5 g dl ; . day 6 after the surgery treatment with sertraline was stopped.
Since its founding in 1990, the Society for Progressive Supranuclear Palsy has provided thousands of patients, caregivers and healthcare professionals support and education on PSP. The mission has always been to increase awareness of progressive supranuclear palsy, advance research toward a cure, educate health professionals and provide support, education and hope for persons with PSP and their families. The vision will always be a world that is free of PSP. To accomplish this ultimate vision, we must work every day to CURE PSP! During the fiscal year that ended June 30, 2005, over 6, 330 individuals, families, foundations, organizations and businesses contributed over $1, 043, 000 to help support the Society's mission. That represents a 6.8% increase in donations above the previous year. In January, it was the Society's privilege to recognize the generosity of our contributors through the publication of the Society's first Annual Report and Honor Roll of Donors. This report was also dedicated to its prime benefactor, Mr. Jay C. Troxel, of Lakeland, Florida. Within its pages, you will find the names of the Society's most recent supporters as well as those in "The Legacy Society" who have provided very significant support over the years. Together, these benefactors have contributed to ensure that the Society continues the work of finding a cure for PSP and providing essential support services to persons with PSP and their caregivers. During FY 2005, the Society implemented a new Strategic Agenda, presenting major directions and objectives to be accomplished through 2010. The Agenda includes bold initiatives in research, outreach, education, communications and organizational effectiveness. An executive summary of the Strategic Agenda is part of the Annual Report. Much has been accomplished by the Society in the area of investigator-initiated research. The Society is committed to basic research and to the development of clinical trials and therapy-oriented pilot proposals. While research that leads to treatment and a cure is of primary importance, the Society continues to be the primary source of education and information for those with PSP and for those who provide medical and health services to these individuals. During FY 2005, the Society reached over 45, 000 people through symposiums, family conferences, support groups, direct mail, and Web site forums. A review of the Society's mission component accomplishments for FY 2005 is included in the Annual Report. As we enter our 16th year of service to patients with PSP, we look forward, with a renewed commitment to our mission and vision and an abiding gratitude for our partners in this caring endeavor.
